PCMH Enrollment Information Center

The Arkansas PCMH program is a voluntary program that requires practices to submit completed enrollment forms to enroll, withdrawal, or update program enrollment.

Enrollment for the 2026 performance year will start on October 6, 2025 and run through November 17, 2025. Enrollment applications will only be accepted during this time.
The program requirements described in the PCMH Manual and Addendum can be found on the AR DHS PCMH website ( Patient Centered Medical Home - Arkansas Department of Human Services ).
Practices applying to enroll in the PCMH program will be required to act in accordance with the rules outlines in the PCMH Manual in exchange for payment.

Enrollment Methods

Practices that are enrolled in the current performance year are eligible to submit their renewal application via the new QCI Program Enrollment portal.
Practices that are not enrolled in the current performance year must submit the completed enrollment application via email to ARKPCMH@gainwelltechnologies.com. Enrollment forms can be downloaded from the links provided below.
